Health Services Information

The following are the requirements needed for each school year:

7th Grade:  Tdap (tetanus) and MCV (meningitis) vaccines due

                   Dental exam (which can be dated up to one year previous from 1st day of school)

11th Grade: Physical exam (can be dated up to one year previous from 1st day of school)

12 Grade: 2nd MCV (meningitis) vaccine (or one dose if first dose was given at/after age 16)

ALL Student Athletes: PIAA Sports Physical (CIPPE)
