Excellence in Action Nomination Form

First Commonwealth Bank has partnered with Bishop McCort Catholic High School (BMCHS) to recognize students, teachers, and staff members who strive for excellence in school and in their community. The First Commonwealth Excellence in Action Award will provide deserving students and/or staff members with a certificate of commendation, a $50 gift card, and a pass to be first in the lunch line for a week at the high school.

Nominations for the award may be submitted by completing the fields below. All recommendations will be sent to the Principal for review. Nominations will be accepted throughout the school year. Click here to view past recipients.
