Name of Event: Senior Retreat/Picnic

Destination: Greenhouse Park

Designated Supervisor of Activity: Mr. Denhard, Ms Piatak, & Mr. Pfeil 

Date & Time of Departure: Students should report to school for HR (8AM) on TUESDAY MAY 28th, Students will attend Mass together at 8:45 AM and then dismiss to Greenhouse Park at 10:30 AM

Date & Anticipated Time of Return: May 28th, Students will leave Greenhouse Park at 2:00PM

Method of Transportation: Students will provide their own transportation to and from Greenhouse Park.  Students are encouraged to car pool, but any student who may need transportation can just come to guidance to arrange transportation assistance.  Please be aware that you are consenting for your student to drive their own vehicle.  

Student Cost: None 

Dress Code: McCort TShirt and long pants for Mass - Shorts & TShirts for Retreat

Notes: Students are encouraged to bring sides dishes for the cookout.  School will provide drinks and food for the grill.  You should bring sports equipment, games, and fishing is allowed.  No swimming!  

If you would like your child to participate in this event, please complete the following statement of consent and release of liability.  As parent or legal guardian, you remain fully responsible for any legal responsibility, which may result from any personal actions taken by the named student.

I hereby consent to participation by my child, in the event described above.  I understand that this event will take place away from the school grounds and that my child will be under the supervision of the designated school employee on the stated dates.  I further consent to the conditions stated above on participation in this event, including the method of transportation.
